What is your new patient procedure?

As an incoming client, you will receive a intake session that consists of a bio-psychosocial and development of a master treatment plan. This process takes approximately one hour and can completed in office or at your home. During this initial session, you will be able to explain to the intake coordinator what you are seeking treatment for, ask any questions and feel open to share any history that will be relevant to treatment. After the bio-psychosocial, you and the intake coordinator will discuss what goals you have for yourself during treatment; this will be taken into account when the therapist completes your treatment plan before your first therapy session.

I don’t have my insurance card with me, can I still receive treatment?

You can provide the intake coordinator with pertinent information so that your insurance coverage can be verified prior to coming for your intake session. Please bring in your insurance card, identification card (such as a drivers license) and your social security card for your client file.

How much will my copayment be?

The copayment is set by your insurance provider based on your plan. Prior to coming in for your intake session, your insurance will be verified and any copays will be discussed prior to making your first appointment.

What should I do if I need to miss an appointment?
If you need to miss an appointment, please give your therapist at least 24 hours notice. You will usually be able to reschedule a new appointment later in the week. However, if you cancel within the 24 hour window or never attempt to cancel or reschedule, a $40 cancellation fee will be collected.
